$703 millionalleged Medicare scheme; approximately $418 million was paid

Prosecutors say defendants in a $703 million Medicare scheme charged in Illinois in 2025 used artificial intelligence to create fake recordings of beneficiaries consenting to receive products.

A recorded consent call is an anti-fraud control; the allegation is that the control itself was manufactured. The case was part of a June 2025 takedown that charged 324 defendants. Everything described is an allegation.
